TUITION APPLICATION

Bend Forest School uses School & Student Services to process tuition applications and determine which tier applicants fall within. We suggest reviewing the following steps well in advance in order to better ensure your application and supporting tax documents are submitted by the term deadline. We guarantee that all financial materials provided remain under strict confidentiality.

 

If your family’s annual household income is $200,000 or above and you only have one child enrolling in a program, please notify us in lieu of submitting the SSS form.

Create an Online Account

 

Click on link below and type in school code when prompted. Our school code is 200642 

 

If access to a computer is limited, please contact us and we'll be happy to assign a support person to assist you in completing all the steps.

Complete the Parent Financial Statement (PFS)

 

Once you have created an online account, you'll complete your Parent Financial Statement (PFS). This statement gathers information about your family size and number of children enrolled, income, and expenses which is used to estimate the total amount your family can contribute towards the cost of tuition. SSS uses this information, in conjunction to the tax information submitted, to determine the actual number the BFS Tuition Committee will use for tier placement. 

Please note that only those within our BFS Tuition Committee will view your application for tier placement. We choose to work with a third party software in order to maintain our families privacy as best we can while ensuring funding is gathered and distributed equitably in our program.

Submit Application Fee Payment

The fee to submit the application is $55, for any number of applicant children. Please contact the Administrative Coordinator to request a fee waiver as needed. Once payment has been submitted, we cannot issue a refund. This is the only fee associated with the TADS application process.

Upload Supporting Tax Documents

After you pay the application fee and submit your PFS, you'll have access to the PFS Online section called "Manage Documents."

 

Please then upload the following required documents to complete your submission: 

  • 2021 1040 with all filed schedules and attachments

  • 2022 1040 with all filed schedules and attachments

  • 2022 Form W-2 (s)

  • 2022 Form 1099 

IMPORTANT: Without these documents we cannot process your application, so please be mindful in allocating enough time to gather them!

Allow Time for Processing

Once you have completed your PFS and uploaded your tax documents, please allow for up to 2 weeks for SSS to process your family application. Once your application has been processed, we will contact you with your tier placement. 

If you would like to apply for tuition assistance after viewing the tuition range your family falls within, please notify us that you'd like to do so and we'll consider your application when distributing what funding is currently available within our equity fund.

SCHOLARSHIPS

Your family could also qualify for a scholarship, in addition to receiving tuition assistance through our affiliated nonprofit organization, Bend Forest School Foundation, if your annual household income falls below the median household income for Bend, OR and/or your family has multiple children enrolled in the program that results in a lower tier placement. Individual scholarship amounts and the number of scholarships awarded are contingent on what funds are available within the foundation for the upcoming school year. To be considered for a scholarship, please contact the foundation's Executive Director for more information.

CHILDREN'S SCHOLARSHIP FUND

Additionally, the Children’s Scholarship Fund-Oregon provides children of low-income families with tuition assistance in grades K-8 and can be obtained in conjunction with a scholarship from Bend Forest School Foundation for families who qualify. CSF - Oregon supports expanding educational opportunities for all children and awards scholarships between $1,500 and $1,700 on average per child per year. Scholarships are capped at $2,000.
